How to read your Health Snapshot

Make sense of your results over time

Your Health Snapshot is a simple picture of how you're doing across a few key areas of your life, tracked over time.

What you're looking at

Each line on the Snapshot is one area of your life, showing movement from “needs support” towards “doing well” over time. As you complete assessments, the lines update to reflect how you're doing.

Good to knowYour Snapshot needs at least two completed assessments before it can show change over time. Until then, it shows your current results as dials.

Seeing each area on its own

Tap an area to open it on its own. You'll see its trend over time, a plain-language rating like “Low,” when it was last updated, and helpful factsheets. “Retake Questionnaire” lets you refresh that area whenever you like.

Opening a single area shows its trend, current rating and factsheets, with the option to retake that questionnaire.

A quick list view

You can also see all your areas together as a list, each with a simple rating, trend and how recently it was updated.

Latest Results lists each area with a rating, trend and how recently it was updated. The coloured bar gives a quick read at a glance.

How your results are shown

Each area has a result label (a simple word or rating) and a position on the line, so you can see both where things are now and how they've moved. You don't need to interpret numbers — the labels and the shape of the line tell the story.

What changes mean

A line moving up shows an area where things are going well; a dip points to an area where you and your clinician might focus next. It's completely normal for different areas to move in different directions.

TipThe Snapshot is a conversation starter, not a score to worry about. Bring anything you notice to your next session.
