Exploring care options and building a care plan

Find care options and plan with your clinician

Within each health area you can explore care options and ask your clinician for input, so you build a care plan together.

Where to find care options

Care options appear in two places: inside each of your health priorities (for example, Social Connectedness), and on your health cards (the cards showing each area's latest result). Open either to see the options relevant to that area, along with your latest result and how it's been tracking.

You can filter the options by Self Care (things you can explore on your own) or Work with Clinician. Links open the resource directly.

Recommended care options show who's supporting them and their status, like “In progress.” Your goals appear lower on the same screen.

Asking your clinician for input

If you'd like your clinician's help with a care option, you can request to work together on it, and you'll see its status (like In Progress or Supporting) as you go. Some options are self-guided (for example, an app you can start on your own), while others are supported by a named clinician.

Together you and your clinician can use these options to agree the next steps that make up your care plan.
